Volume of distribution (Vd) is a hypothetical volume that relates the amount of drug in the body to the plasma concentration. It does NOT represent a real anatomical space; rather, it indicates how extensively a drug distributes into tissues relative to plasma.

where C₀ is the initial plasma concentration immediately after IV administration.

Low Vd (e.g., 3–5 L): The drug is largely confined to the plasma compartment. Examples: warfarin (highly protein-bound), heparin (large polar molecule).
Medium Vd (e.g., 10–20 L): Distribution into extracellular fluid. Examples: gentamicin, atenolol.
High Vd (e.g., > 50 L): Extensive tissue binding – drug leaves plasma and accumulates in fat, muscle, or organs. Examples: digoxin (Vd ~ 500 L), amiodarone, chloroquine.

Clinical Significance: Vd determines the loading dose. A large Vd requires a larger loading dose to achieve a rapid therapeutic effect. Conversely, drugs with a small Vd do not need loading doses – they reach steady-state quickly.
